Madden Mental Health Center Discharge Support

The NAMI Transitions Team is committed to helping you enjoy a successful, healthy and supported transition back to the community. We work with anyone currently at John J. Madden Mental Health Center in Hines, Illinois who is getting ready to discharge. We will follow your progress and provide help as needed to avoid readmission to the hospital for preventable causes. Services are free of charge.

How to Get Started?

There are only two simple steps to getting started with this program:

  1. You must have a desire to live in the Chicagoland Area
  2. Let your Madden Mental Health Center social worker know that you are interested in participating

We Will Connect You With

  • A dependable food supply
  • Step-down services as needed (IOP, PHP)
  • Ongoing medication management
  • Ongoing mental health support providers
  • Community peer support programs (Living Room, Drop In Centers)
  • Jobs support through NAMI IPS services
  • SSDI and SSI benefits support

Direct Services Available

  • Risk assessment and safety planning
  • Clothing, cell phone, toiletries, personal items, bus passes

What Will You Gain?

After discharging from Madden Mental Health Center, you’ll work with our peer recovery specialists who understand what you’re going through because they’ve been there themselves. Many participants transition to our other programs including the Living Room for ongoing peer support or the Drop-In Center for daytime socialization and skill-building. If you’re interested in employment, we can connect you with Workforce Development services.
